• Download/Save the form to your computer
• Open the form in Adobe and fill out the applicable fields
• Once the form has been completed, you can email the form as an attachment to: credit@rell.com or you can hit the “Submit Form” button at the bottom of the first page. Selecting that button will open your email platform and automatically attach the form to a new message.  Don’t forget to hit send so the message sends from your email.
